Model Release Form

There are many rules regarding when you need permission to record individuals, the likeness of individuals in particular settings or environments. Always use a model release form when you record a person in video or still photography or any media likeness, including painting. A general rule is if you can accurately see their face on the finished product you must have them sign a model release form.

Payment

Fill in this if you are compensating your model either in the form of money or a copy of the finished work. If you are not compensating your model leave this section blank.

In return for the use of their name, picture, portrait or photograph in all forms and media and in all manners, including composite or distorted representations, for advertising, trade, or any other lawful purposes.

The Model waives the right to inspect or approve the finished work, including written copy that may be created in connection therewith.

The Model is of legal age and has read this release and is familiar with its contents.

• If the Model is underage, the parent or guardian of the minor must sign a contract stating that they understand the above, and have the legal authority to execute the release.  Be sure to delete the statement above that they are of legal age.
